Consent form. Forumlario de consentimiento.

Dear Eater Families,

As early as next week, JW Eater will be able to provide on-site PCR/Saliva testing for COVID. This is the same testing program that is currently in place for University of Illinois staff and students. In order to keep our school open, the more students who participate in this weekly testing, the better success we will have. Please consider voluntarily consenting to allow your student to participate in the testing.

If your student is participating in a school sponsored activity, including Scholastic Bowl, Girls Basketball, and Cheer participation in the testing is NOT VOLUNTARY, but we must still have your written consent. For these students, we must have the completed form by next Friday, April 2, 2021. Once we begin testing, we plan to test these students two times per week. If your student does not participate in testing, we will consider your student ineligible.

Attached here is the consent form giving us permission to test your student at least weekly. This form is in English and Spanish, but you only need to complete the form in one language. You may return this form electronically or send a printed copy to school. We will send hard copies of the consent form home with students next week.

Queridas familias de Eater,

Tan pronto como la próxima semana, JW Eater estará disponible para brindar PCR/pruebas para COVID por saliva en el establecimiento. Este programa de pruebas es el mismo que se está utilizando actualmente para los estudiantes y empleados de la Universidad de Illinois. Como orden de mantener nuestra escuela abierta, mientras más estudiantes participen de estas pruebas semanales, mayor será nuestro éxito. Por favor, consideren voluntariamente autorizar a su estudiante a participar en estas pruebas.

Si su estudiante está participando en una actividad escolar, incluyendo "Scholastic Bowl", "Girls Basketball", y “Cheer” su participación en estas pruebas no es voluntaria, sin embargo necesitamos su consentimiento por escrito. Para estos estudiantes, debemos tener el formulario completado para este próximo viernes, 2 de abril de 2021. Una vez comencemos con estas pruebas, planeamos examinar a estos estudiantes dos veces por semana. Si su estudiante no participa de estas pruebas, lo consideraremos un estudiante inelegible.

Adjunto está el formulario de consentimiento que nos da permiso para evaluar a su estudiante por lo menos semanalmente. Este formulario está en inglés y español, pero solo necesita completar el formulario en un solo idioma. Puede devolver este formulario electrónicamente o enviar una copia impresa a la escuela. Enviaremos copias impresas del formulario de consentimiento a la casa con los estudiantes la próxima semana.
